The Guelph Wellington Ontario Health Team (OHT) is one of 58 Ontario Health Teams across the province. The GW OHT is a collaborative of local health and social care partners who are working to strengthen local services, making it easier for patients to navigate the system and transition between providers. The shared purpose of GW OHT partners is to “Advance integrated care through partnership and collaboration to improve the health of the population of Guelph Wellington.”
