The ECHO Care of the Elderly (COE) Long Term Care (LTC) Series utilizes the “all teach, all learn” model of knowledge dissemination and capacity building to connect LTC team members with geriatric specialists for collaborative and case-based learning. 

*The program is not offered by Centre for Resilience, Learning & Growth – it is led by a community partner. However, we wanted to share information about the program as it may benefit the communities we work with.

Who is this for?

For physicians, nurse practitioners, nurses, social workers, physiotherapists, occupational therapists, personal support workers, and all health care professionals working in Long Term Care in Ontario.

Extension of Community Healthcare Outcomes (ECHO) at University Health Network (UHN) is a supportive, virtual learning community for primary care providers (PCPs) to enhance their skills and confidence by sharing of best practices and practical advice for real patient cases. They aim to build capacity through a virtual community of practice for health care providers in Ontario that will increase access to specialist healthcare, improve patient outcomes, and create health equity.
